IPHI Data Sharing in Practice
Chicago Regionwide Community Information Exchange
IPHI partnered with Chicago and Cook County to create the Chicago Regionwide Community Information Exchange (CIE), which connects healthcare providers, social services, and community-based organizations to advance health equity. Through the CIE, case workers and healthcare providers across the region can better coordinate complex client care for people experiencing homelessness, improve access to behavioral health resources for people across the region, and more.
Community Health Needs Assessment (CHNA)
The Alliance for Health Equity’s Community Health Needs Assessment integrates public health data, community input, and existing research to document health status in Chicago and Suburban Cook County. The 2025 CHNA was developed through a collaborative, equity-driven approach led by IPHI adapting components of the County Health Rankings & Roadmaps and a MAPP 2.0 framework. This included 40 focus groups across the county and a survey of nearly 2,000 people designed to oversample the populations hardest hit by health inequities.
Data Across Sectors for Health
For nearly a decade, IPHI worked with the Michigan Public Health Institute on Data Across Sectors for Health (DASH), a national initiative elevating models, principles, and practices that support equitable, multi-sector data sharing.
The legacy of this work will live on in the DASH Knowledge Base, a searchable repository of articles, briefs, toolkits, webinars, and podcasts.
