Innovative Partnerships: Collaborating for Healthy and Equitable Communities
What happens outside the walls of a health facility has more impact on population health than what happens inside. Hospitals and health systems are partnering with each other, and building relationships with new partners to begin to address unmet needs in communities to improve health. Leaders from a Wisconsin tribal health center/school partnership to address trauma and adverse childhood events, a Chicago-area multi-hospital, multi-stakeholder social determinants of health collaborative, an Indiana collaboration addressing the County Health Rankings, and a health system working to mitigate the opioid epidemic will discuss partnering across sectors to address critical community health issues that can’t be solved alone.
