Continental breakfast and lunch will be provided on all meeting days. Participants will be responsible for their own dinners.
Please dress business casual. You may wish to bring a sweater or jacket since the meeting rooms can get cold at times.
Cancellations, Substitutions, and Refunds
Registrations and cancellations received after November 10, 2017 are not refundable. If you need to cancel after November 10, 2017, please call 312-850-4744. One attendee may be substituted for another at any time by calling 312-850-4744.
We are pleased to offer a limited amount of funding for scholarships. Please note that scholarships are available to assist with program enrollment costs ONLY and are not applicable for travel, food, or accommodation costs associated with attending the program. To apply for scholarships, complete the online Scholarship Application. The application deadline for scholarships is November 1, 2017 and are accepted on a rolling basis.
Scholarships will be prioritized for registrations from community-based organizations with budgets less than $2 million, participants from local and state health departments, Federally Qualified Health Centers, and other small, rural, or critical access hospitals with limited funds for participation.
