IPHI co-leads Data Across Sectors for Health, which is a partner of All In: Data for Community Health, a national network of projects with the common goal of improving multi-sector data sharing and collaboration.
All In: Data for Community Health is hosting the first in series of project showcase webinars highlighting insights and lessons from local collaborations integrating multi-sector data to improve community health.
This webinar will feature opening remarks from Karen DeSalvo, former Acting Assistant Secretary for Health and National Coordinator for Health IT, followed by presentations from data sharing projects in three communities.
Each project is designing and implementing a user-centered, interactive application that leverages data from EHRs and other sources to improve the health of under-resourced populations. Presenters will share strategies for determining the right data sets to incorporate, engaging stakeholders from multiple sectors, and designing platforms that are useful and effective for end users, including patients, caregivers, and care coordinators.
- Essential Access Health is building an application for teen pregnancy prevention that will interface with multiple EHRs using FHIR standards and allow teens to fill out assessments and access resources.
- Children’s Comprehensive Care Clinic has developed a prototype for an application that integrates data from patient EHRs, school records, claims, etc. to improve care coordination for families of children with medical and behavioral complexity.
- HealthInfoNet is using predictive analytics to identify high-risk patients and incorporating social determinant data into its statewide health Information exchange (HIE) to provide a holistic perspective on patient care coordination for case managers and clinical staff.
