Midwest Forum Call for Abstracts Due Aug. 11, 2017

The Midwest Forum on Hospitals, Health Systems and Population Health: Partnerships to Build a Culture of Health will bring together organizations and key stakeholders dedicated to improving the health of all Americans. The goal of the meeting is to engage attendees in a broad dialogue across sectors to solve complex issues to improve the health of communities and integrate clinical and community health and prevention strategies. Participants will share innovative models that are working and inspire the audience to take action to help transform the existing system of care in the United States to a system that strategically plans for and supports the health of its population. Building on health equity as the core forum theme, topics include:

  • The policy environment for population health and health system transformation
  • Using data for care coordination and population health
  • Aligning financial incentives and payment for population health
  • The role of purchasers and payers in population health
  • Integrating physical and behavioral health
  • Developing clinical/community linkages
  • Connecting health with community development
  • How to engage consumers and patients in population health
  • Collaborating on and leveraging community health needs assessment and planning
  • Partnerships for health equity and social determinants of health
  • Promoting livable communities
  • Collaborating for community health and equity in rural areas

The Midwest Forum on Hospitals, Health Systems and Population Health is seeking abstracts from stakeholders in Illinois, Indiana, Iowa, Ohio, Michigan, Minnesota, and Wisconsin to highlight successes and lessons learned related to the topics above. Concurrent breakout sessions will be 75 minutes. Proposals can be for either 1) a 20 minute presentation that will be combined with 2 other presenters during the concurrent session, or 2) the full 75 minute workshop in which the proposer will organize multiple presenters and/or facilitate interactive dialogue among participants. Abstracts submissions are due Friday, August 11, 2017, 11:59 pm CT.

All abstract submissions should be emailed to MWFabstracts@iphionline.org and must include the following information:
1. Lead presenter’s name
2. Co-presenter name(s), if applicable
3. Organization(s)
4. Contact information, including phone and email address
5. Time requested: ___ 20 minute presentation ___ 75 minute workshop
6. List 2-3 learning objectives for the presentation/workshop
7. A brief summary of the presentation/workshop (up to 200 words). If you are proposing a workshop, please include
information about the types of presentations/presenters included and any interactive elements included in the
session.
8. Speaker bio(s) (no more than 100 words each)

Abstract Review:
A group of Midwest Forum Planning Committee members will review all submitted proposals and will make selections based on the following: 1) Proposal content aligns with one of the several topics and themes for the Midwest Forum; 2) Geographic diversity of the speakers across the Midwest; 3) Sector diversity- ensuring multiple sectors are represented in presentations and workshops; 4) Interactive nature of the workshops- ensuring workshops are engaging for multi-sector stakeholders

The lead presenter will be notified of acceptance by email no later than Sept. 8th, 2017. All presenters will be required to register for the Midwest Forum no later than Nov. 15, 2017.
