A presentation by Jim Hester, PhD, Principal of Population Health Management Group and former director of the Population Health Models Group at the Centers for Medicaid and Medicare Innovation, will focus on the concept of Community Integrated Service Delivery and discuss how to build a sustainable financial model for population health.
