Due to changes in the US health care delivery system as a result of the Affordable Care Act, there is an opportunity to develop a truly integrated system to build healthy communities and address the social and economic drivers of health. A presentation by Jim Hester, PhD, Principal of Population Health Management Group and former director of the Population Health Models Group at the Centers for Medicaid and Medicare Innovation, will focus on the concept of Community Integrated Service Delivery and discuss how to build a sustainable financial model for population health.
Michael Gelder, Senior Health Policy Advisor to Governor Pat Quinn and Executive Director at the Governor’s Office of Health Innovation and Transformation, Leticia Reyes-Nash, Division Chief at the Illinois Department of Public Health and Craig Conover, MD, State Epidemiologist at the Illinois Department of Public Health will hold a panel to provide an overview of innovative systems change in Illinois.
